STEP 1

Fill out the on-line form: Medical History/Lifestyle Form

STEP 2

Our office will contact you to schedule a consultation. We will assist you in ordering the necessary blood tests on-line. This can be done at your convenience 24 hours a day.

Fill out the on-line form: Blood Work Order Form. We have blood testing locations across the country, usually within 10 minutes of your home. Click on the image to the right to find the lab location in your area.

STEP 3

Complete a Physical Exam. After receiving your blood profile, medical history form and physical exam, you will receive an assessment from our physician. This assessment will be used to bring your hormone levels to the optimal range if needed also you will be referred for personal training, nutrition guidance and weight loss program.

FOLLOW UP

This will consist of additional blood work and a scheduled appointment at 6-week and 6-month intervals upon start-up to re-test blood levels, then annually, to adjust dosage and review details of your program to ensure you are achieving the maximum results. In addition, you will receive phone and email support as needed.
